The Oracle location is one of Horizon's neighborhood care centers. Horizon Health and Wellness is a nonprofit community health center that has served Arizona for more than 40 years, with care centers in Apache Junction, Casa Grande, Chandler, Florence, Globe, Oracle, Phoenix, Queen Creek and Yuma. Its integrated approach brings a team of health care professionals together to treat the whole person, mind, body and spirit, often in one location. Horizon is a federally qualified health center (FQHC) and a certified Behavioral Health Home. Services across Horizon include family medicine and primary care, pediatric care, behavioral health and counseling, psychiatric services, substance use treatment, crisis services, domestic violence services, group therapy, HIV services, intellectual and developmental disabilities services, lab services, pharmacy, residential treatment, supported employment, supportive housing, a medical weight loss program and telemedicine.
